Please continue to keep Family Care OHP as it is the only plan we are currently contracted with.
If Family Care does close after Jan 2018 you will be changed to Healthshare Care Oregon, which MHW is working on contracting with. At no time will you be without insurance.
Please keep your regularly scheduled appointments and please make sure you come in to see your provider in December of January so we can plan ahead of any changes.

OTHERWISE ALL OTHER INSURANCE PLANS WILL STAY THE SAME.
